The lockout program provided by the employer did not include, but not limited to:   1/ the program was not documented to show the limitation of service to be provided by the M & R Operators and the maintenance mechanics.   2/ the program did not have specific steps documented to ensure the equipment was adequately de-energized prior to being serviced or repaired.  3/the program did not have a documented means to control the use and identity of locking devices.  4/the program did not identify designated personnel.    Equipment include: M & R Machines and associated electrical equipment.    Observed on or about 04/15/2015.
