Insulation Storage Loft - On or about April 17, 2015, the staircase utilized for access to the insulation storage loft area was not provided with handrails on either side of the open staircase.   Instance b. Title department - On or about April 17, 2015, the staircase in use for emergency exit was not provided with a handrail on the open side of the staircase.
