29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Worksite located at 13104 N MacArthur. Oklahoma City, OK 73142. On or about April 16, 2015 and at times prior there to, employees were observed performing framing work at a height above 6 feet without the use of fall protection. The employees were exposed to a fall hazard.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   Worksite located at 13104 N MacArthur. Oklahoma City, OK 73142. On or about April 16, 2015 and at times prior there to, employees were observed working in areas with a possible danger of head injury from falling objects without the use of protective helmets. The employees were exposed to struck-by hazards of the falling objects.
