29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:    On the eastside of the building at 1359 Lomaland Dr., an employee was observed using a DeWALT angle grinder (model DW 840, Serial No. 691712)/9-inch cutting wheel with the safety guard removed. The employee used the angle grinder to cut through brick and cement block making cutouts of approximately 16 x 22 inches on the exterior walls of the building.

29 CFR 1926.403(b)(1)(iii): Safety of equipment was not determined on the basis of electrical insulation:    On the eastside of the building at 1359 Lomaland Dr., the employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees in that the insulation of a 100-foot blue 12/3 AWG extension cord (used to power the DeWALT angle grinder, Model DW 840, Serial No. 691712) was cut through exposing bare conductors.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:    On the eastside of the building at 1359 Lomaland Dr., an employee was observed using an Ingersoll Rand/VR 642C Telescopic forklift to lift a work platform and move it horizontally at heights of approximately 17 to 21 feet while the platform was occupied.

The Platform (attached to the Ingersoll Rand/VR 642C Telescopic forklift) reached heights of approximately 17 to 21 feet, had three vertical walls 48 inches high with the front of the platform left open (no wall).

The employee was not trained in the recognition and avoidance of hazards associated with a work platform that did not meet the requirements for capacity, construction, access, use, and fall protection.
