29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) Fabricated scaffolding systems: On or about April 15, 2015, the employer allowed the use of scaffolding systems with no guardrails erected on the ends which exposed employees to falls up to 20-feet to the ground below.   b) Fabricated scaffolding systems: On or about April 15, 2015, the employer allowed the use of platforms bridging the gap between perpendicular scaffolding systems with no guardrails erected which exposed employees to falls up to 20-feet to the ground below.    c) Fabricated scaffolding systems: On or about April 15, 2015, the employer allowed the use of scaffolding systems with only cross braces erected which exposed employees to falls up to 20-feet to the ground below.
