29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  (a)  At the Lighthouse of Howard jobsite in Section / Area A; a flexible cord (extension cord) was plugged into a temporary electrical outlet in order to power a Boschhammer when needed.  The flexible cord was frayed/worn at the male end.
