OSH ACT of 1970 Section (5)(a)(1): Executive Order 12196 Section 1-201(a): The head of the agency did not furnish to employees places and conditions of employment that are free from recognized hazards that are causing or are likely to cause death or serious physical harm in that employees were exposed to hazards associated with the Slitter Winder Machine and no estop.  On or about 4/20/15 in building # 1: Employees were exposed to a laceration and amputation hazard from the Slitter Winder Machine (serial no. 9841) ; the red push buttons were not labeled and no estop on the green control panel and not readily accessible.   Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the National Fire Protection Association (NFPA) 79, Electrical Standard for Industrial Machinery, 2012 Edition. Stop and Emergency Stop push buttons shall be continuously operable and readily accessible in accordance with the guidelines set forth in NFPA 79, Article 10.7 .1 Location and Operation.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:  On or about 04/20/15 located in Building # 2  a) Exit light was out for the exit door on the East side welding area. b) Exit light was out for the exit door on the East side front wall area. c) Exit light was out for the exit door on the South East side front wall area office door. d) Exit light was out for the exit door on the South East side front wall area office door inside. e) Exit light was out for the exit door on the North East entrances door.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:  On or about 4/20/15 located in Building 1, the Slitter Winder Machine (SWM) ; the employer failed to ensure that the rotating shafts was guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operations a 21 inch high x 31inch wide opening not guarded.
