29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  Engine Room of Vila Novo do Corvo II Scalloping Vessel:    Employer did not adequately guard the rotating fan blades of the rear starboard-side generator, at the point of operation, exposing employees to amputation hazards. On or about March 21, 2015, the right hand of an employee who was working in the vicinity of the generator slipped into the exposed rotating blades crushing the right forefinger and part of the middle finger.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  Establishment:  Employer did not report an amputation injury within 24 hours of the time of injury. On or about March 21, 2015, an employee who was working in the vicinity of a generator inadvertently slipped his hand into exposed rotating blades of the generator crushing the right forefinger and part of the middle finger resulting in amputation.
