OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    (a)  Throughout the ammonia refrigeration process, on or about April 13, 2015, and at times prior to, the employer did not develop a procedure for receiving fresh or make up ammonia per 29 CFR 1910.119(f)(1), exposing employees to chemical hazards.    (b)  Throughout the ammonia refrigeration process, on or about April 13, 2015, and at times prior to, the employer did not ensure that the ammonia receiver vessel was designed and stamped with the minimum design metal temperature per IIAR (International Institute of Ammonia Refrigeration) 2 - 2008 Section 9.1.5, exposing employees to chemical hazards.    (c)  Throughout the ammonia refrigeration process, on or about April 13, 2015, and at times prior to, the employer did not develop a procedure for draining oil from the ammonia receiver outlining precautions to prevent exposure per 29 CFR 1910.119(f)(1)(iii)(B) and IIAR 2 - 2008 Section 14.2, exposing employees to chemical hazards.    (d)  In the condenser room and main production area, on or about April 13, 2015, and at times prior to, the employer did not repair damaged piping insulation that was allowing the build up of ice per 29 CFR 1910.119(j)(5) and IIAR 109 Sections 4.7.4 and 4.7.5, exposing employees to chemical hazards.    (e)  Throughout the ammonia refrigeration process, on or about April 13, 2015, and at times prior to, the employer did not inspect or replace safety valves at regular intervals per 29 CFR 1910.119(j)(4)(ii), exposing employees to chemical hazards.    (f)  Throughout the ammonia refrigeration process, on or about April 13, 2015, and at times prior to,   the system information did not include the maximum ammonia inventory of the system, resulting in a chemical hazard.
