29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Worksite, West Side of Building: Employees were using Doal Band Saw Model #1612-0, Serial #209-66971 to cut metal pieces with approximately 16" of the blade unguarded and exposed; on or about 4/22/15.    Note:  In addition to abatement documentation, the employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

The distance between the abrasive wheel periphery and the adjustable tongue at the top was approximately one inch; on or about 4/22/15.
