OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to a masonry wall collapse:       a) Exterior Building Wall: Employees were exposed to a struck by hazard from a concrete wall collapse. During the course of demolition activities, an inspection by a competent person to determine structural conditions and possibility of an unplanned collapse was not performed. On 4/22/2015, an employee sustained fatal injuries from a cut masonry wall that collapsed during demolition activities.

Prior to demolition activities, an engineering survey by a competent person to determine structural conditions and possibility of an unplanned collapse was not performed. On 4/22/2015, an employee sustained fatal injuries from a cut masonry wall that collapsed during demolition activities.
