On April 13, 2015, the workers on the 2nd floor level were not protected from falling hazards by use of fall protective devices such as body harnesses, guardrails and or safety net systems.   b. On April 13, 2015, the workers on the retaining walls were not protected from falling hazards by use of fall protective devices such as body harnesses, guardrails and or safety net systems.
