29 CFR 1910.178(p)(1):   A powered industrial truck was found to be in need of repair, defective, or in any way unsafe, and was not taken out of service until it had been restored to safe operating condition.  Employees working at Jolly Jumps of St. Louis LLC were operating a defective Big Joe High Lift Walk Behind Straddle Truck, Model PDI-20-T9/ Serial # 357848.  The lift truck had an inoperable high speed control switch.  The high speed control switch was hanging on the lift truck.

29 CFR 1910.178(l)(1)(ii):   Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training.  On 4/21/2015, Jolly Jumps of St. Louis LLC had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training.  Two employees were  operating a powered industrial truck without required training.
