29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a)  At 1513 Camillia Street in Farmington, MO - On or about 04/22/2015, employees were working on a residential roof (steep roof) approximately 14ft above a lower solid concrete drive-way and sidewalk without having any fall protection system in place, exposing employees to elevated fall hazards.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a)  At 1513 Camillia Street in Farmington, MO - On or about 04/22/2015, employees were using a "Werner" 32ft fiberglass extension ladder to access a residential roof (steep roof) surface, approximately 14ft (from roof edge) above an earth surface, where the ladder side rails extended only 1 1/2ft above the roof surface, exposing employees to elevated fall hazards.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  a)  At 1513 Camillia Street in Farmington, MO - On or about 04/22/2015, employees were using a "Werner" fiberglass 32ft extension ladder to access a residential roof (steep roof) surface, approximately 14ft (from roof edge) above an earth surface, where the ladder side rails extended only 1 1/2ft above the roof surface and the employer had not provided training to enable each employee to recognize hazards associated with ladders, exposing employees to elevated fall hazards.
