29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded, so as to prevent the operator from having any part of his body in the danger zone during the operating cycle:   a)  Welding Department, Line "C".  On or about April 20, 2015, and at times prior thereto, employee(s) were exposed to amputation hazards from the lack of a guard at the point of operation on the one-button Daihen electrode pedestal welder, while welding metal parts.   b)  Welding Department, Line "D".  On or about April 20, 2015, and at times prior thereto, employee(s) were exposed to amputation hazards from the lack of a guard at the point of operation on the two-button Daihen electrode pedestal welder, while welding metal parts.
