29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):     a) ) Grimm Industries / 7070 West Ridge Road, Fairview, PA 16415: On or about April 25, 2015 and times prior thereto, employees periodically performed servicing, maintenance and repairs on various machines, including, but not limited to, milling machines, lathes, saws, pickers and molders and associated equipment. The procedures did not include specific information regarding the specific procedural steps for notifying affected personnel prior to beginning the servicing, maintenance, or repairs; the identification of the energy sources of the machine/equipment; specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; specific procedural steps for testing machines and equipment to verify the de-enerergization; specific requirements for inspections of work area prior to reenergizing equipment; and specific requirements for notification of employees prior to reenergizing equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Grimm Industries / 7070 West Ridge Road, Fairview, PA: On or about April 24, 2015 a Roll-In Bandsaw in the tool makers area did not have guarding to protect the unused portion of the saw blade during cutting operations.
