The procedures did not include specific information regarding the specific procedural steps for notifying affected personnel prior to beginning the servicing, maintenance, or repairs; the identification of the energy sources of the machine/equipment; specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; specific procedural steps for testing machines and equipment to verify the de-enerergization; specific requirements for inspections of work area prior to reenergizing equipment; and specific requirements for notification of employees prior to reenergizing equipment.
