29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:  a) Workplace, Rim Straightening Dept.- An electric cord from a portable floor fan in operation had damaged external sheathing which exposed energized wires; on or about 4/27/15.  Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Workplace, Powder Coat Department- Powder Coat Machine was plugged into a metal quad box that was not mounted to a wall, beam, etc.. The Quad box was not used in accordance with instructions; on or about 4/27/15.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Workplace, Rim Cleaning Area- Employees exposed to corrosive materials such as Zep-A-Lume Aluminum Cleaner, which contains sulfuric acid, were not provided with suitable facilities for quick drenching or flushing of the eyes; on or about 4/27/2015.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
