29 CFR 1910.1001(k)(1): The employer did not maintain all surfaces as free as practicable of ACM waste and/or debris and accompanying dust:  (a) Washington Headquarters Services - Raven Rock Mountain Complex - The employer did not develop and implement appropriate housekeeping procedures for the chysotile identified in settled dust samples collected from several elevated horizontal locations such as; but not limited to, pipe runs, shelving, and inner office roofs throughout the facility and on the roof, on or about May 28, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1020(e)(1)(i): When an employee or designated representative requested access to a record, the employer did not provide access within 15 working days and did not apprise the employee or designated representative the reason for the delay and earliest date when the record can be made available:  (a) Washington Headquarter Services - Raven Rock Mountain Complex - The employer did not provide the records requested on April 10, 2015 until May 11, 2015.  (b) Washington Headquarter Services - Raven Rock Mountain Complex - The employer did not provide the records requested on July 23, 2015 until August 31, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
