29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor was the employee provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    On or about April 27, 2015, an employee was working on a flat roof, approximately 27 feet above the ground without fall protection.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date completed, method of abatement, and states employees and their representatives were informed of this abatement.

On or about April 27, 2015, the employer did not provide training that would enable employee to recognize the hazards of falling and the procedures to minimize the hazard.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date completed, method of abatement, and states employees and their representatives were informed of this abatement.

On or about April 27, 2015, employees used a 70 inch stepladder in the closed position, leaned against an exterior wall and positioned on a wooden pallet to enter the residential construction site. 29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways.  On or about April 27, 2015, employees did not  receive training to minimize the hazards associated with improper ladder use such as but not limited to using a stepladder in the closed position.  The employer stated no training in ladder safety was provided.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date completed, method of abatement, and states employees and their representatives were informed of this abatement.
