29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.      a) Groundcovers Greenhouse & Nursery, Inc. at 4301 E Iliff Ave., Denver, CO 80222:  On or before April 22nd, 2015, employees assisting customers in the parking lot were exposed to tripping hazards in that when concrete parking blocks become displaced the rebar-mounting stands up several inches from the ground and creates a tripping hazard.  Prior to the inspection an employee had sustained a significant injury after tripping on some of the exposed rebar.  This condition exposed employees to tripping hazards.

29 CFR 1910.23(d)(1)(iv): Every flight of stairs having four or more risers, more than 44 inches wide but less than 88 inches wide was not equipped with one hand rail on each enclosed side and one stair railing on each open side:    a) Groundcovers Greenhouse & Nursery, Inc. at 4301 E Iliff Ave., Denver, CO 80222:  On or before April 22nd, 2015, employees traveling between the nursery and greenhouse areas used a flight of stairs which was 82 inches wide and was not equipped with hand rails and stair rails.  This condition exposed employees to trip and fall hazards.

This condition exposed employees to potential corrosive chemical hazards.

The employer had not ensured that all employees using such chemicals had received appropriate hazard communication training which included discussion on corrosive hazards.
