29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to remove or bypass a guard or other safety device;     a)  Shop, 2419 Lemoine Avenue, Fort Lee, NJ: Employer did not utilize lock out tag out procedures while an employee was servicing an air compressor. The air compressor started unexpectedly resulting in the employee sustaining an amputation, on or about 4/28/15.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a)  Shop, 2419 Lemoine Avenue, Fort Lee, NJ: Employer did not have an energy control procedure for employees servicing and maintaining equipment such as but not limited to an air compressor, on or about 4/28/15.
