29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent): a) Loading dock, 2500 83rd Street, Suite 2W, North Bergen, NJ 07047. Employees were exposed to fall hazards of four 4 feet when using a plywood ramp from the loading dock onto a pile of pallets in order to empty spoiled produce into the dumpster. Condition noted on or about 4/28/2015.

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points. a) Rear building exit route, 2500 83rd Street, Suite 2W, North Bergen, NJ 07047. The exit route at the rear of the building was obstructed with a disconnected HVAC unit, metal ramp, and pallets reducing the exit width to approximately 14 inches. Condition noted on or about 4/28/2015.
