29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  a. On or about April 28, 2015, at the above addressed jobsite, an employee on an aerial lift (Genie, S-85) was not wearing a body belt or full body harness. The employee was thereby exposed to the hazards associated with a fall of approximately fifteen (15) feet.

29 CFR 1926.502(d)(17): The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head:  a. On or about April 28, 2015, at the above addressed jobsite, an employee on an aerial lift (Genie, S-85) was wearing a body harness in which the attachment point was not located in the center of the back, thereby exposing the employee to the hazards associated with falls.
