29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a) Plum Grove Nursing and Rehab, LLC dba Aperion Care Plum Grove, Palatine, Illinois did not provide employees the required training on the revised elements of the Hazard Communication Standard for labels (e.g., pictograms and signal words) and the new format of the Safety Data Sheet (SDS) (i.e., section order and types of information contained within each section of the SDS), which was due December 1, 2013.  Affected employees, including those in housekeeping, maintenance, laundry and kitchen, are exposed to hazardous chemicals, such as Spartan Chemical Company Super HDQ Neutral (containing didecyl dimethyl ammonium chloride & alkyl dimethyl benzyl ammonium chloride), Edward Don and Co 2390 Hy-Shyne W B Stainless Steel Cleaner & Polish 1J660 (containing butane, propane & petroleum naphtha), Ecolab Laundry Destainer (containing sodium hypochlorite) Simoniz USA No Odor Stripper (containing monoethanolamine), and Simoniz USA Blend Rite Quat 256, a germicidal disinfectant (containing quaternary ammonium chloride compound).  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
