29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel:   Employees were exposed to struck-by hazards in that the employer allowed employee to operate the forklift with a load of palletized cardboard boxes stacked to 6'2" above the forks.  Employee operated the forklift in the forward direction for approximately 5 feet while the load blocked/obstructed the operator's view.
