Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.     (a) The employer failed to ensure that each employee conducting roofing work at 6120 South Howell Avenue in Milwaukee, Wisconsin, were protected from falling to the lower level by the use of a fall protection system.  This exposed employees to a fall hazard of approximately 14.6 feet.
