29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Employees were observed roofing a steep-sloped rooftop with a pitch of 9:12 at heights greater than 10 feet from ground level without any means of fall protection used, exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Employees were observed using portable ladders to access the rooftop of a residence, with the ladder side rails not extending at least 3 feet above the rooftop landing, exposing employees to fall hazards.
