High Street, Potosi, MO 63664, on or about 4/29/2015, at this work site, roofers were actively installing new roof shingles, operating pneumatic nail guns without any form of eye or face protection. The workers were exposed to a struck-by hazard.

High Street, Potosi, MO 63664, on or about 4/29/2015, at this work site, workers were observed on a 4:12 pitch roof, approximately 12 feet eave to ground, actively installing new roof shingles and were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems. The workers were exposed to a fall hazard.

High Street, Potosi, MO 63664, on or about 4/29/2015, at this work site, workers were observed using a Werner Aluminum Extension Ladder, MODEL D1520-2, Type 1A, for access to an upper surface; the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. The workers were exposed to a potential fall hazard.
