29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:  a) On or about April 29, 2015 employee(s) accessing the top of machine 105 to change out product tubes during product change-overs were exposed to a fall hazard of over 6 feet on all unprotected sides.   b) On or about April 29, 2015 employee(s) accessing the top of machine 106 to insert a product probe into the product infeed tube were exposed to a fall hazard of over 6 feet on all unprotected sides.   c) On or about April 29, 2015 employee(s) accessing the top of machine 107 to insert a product probe into the product infeed tube were exposed to a fall hazard of over 6 feet on all unprotected sides.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about April 29, 2015 employees removing boxes from the glue machine were exposed to pinch point hazards created between the boxes and the downward stroke of the ram.
