29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) 60 West Mountain Rd., Worcester, MA: On or about April 3, 2015 the employer failed to ensure that each employee used the appropriate eye protection when exposed to eye hazards.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.305(g)(1)(iv)(A): Temporary wiring  was used as a substitute for the fixed wiring of a structure:    a) 60 West Mountain Rd., Worcester, MA: On or about April 29, 2015 a black extension cord was used as a substitute for fixed wiring to allow for access to an electrical outlet that was attached to the ceiling more than 20 feet above the working surface.
