29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Third floor top plate, new townhome construction located at 320 Rose Garden Lane, Alpharetta, GA 30009: Employee was exposed to fall hazards of approximately 22-feet, 9-inches to the lower level while installing 3/4-inch plywood subflooring and framing the third floor walls with no fall protection system in place. The employer allowed employees to work without providing guardrails or personal fall arrest systems.   (b) Third floor, new townhome construction located at 310 Rose Garden Lane, Alpharetta, GA 30009: Employee was exposed to fall hazards of approximately 22-feet, 9-inches to the lower level while installing 3/4-inch plywood subflooring and framing the third floor walls with no fall protection system in place. The employer allowed employees to work without providing guardrails or personal fall arrest systems.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  (a) Temporary electrical service, new townhome construction located at 320 Rose Garden Lane, Alpharetta, GA 30009: The employer allowed employees to use two 50 ft. extension cords that were missing ground pins which were providing power to a Makita Circular Saw and a Jenny Air Compressor.
