(a) Location - 11 Broadway Avenue, Rockport, MA:  On or about 4/27/2015, employees working from a ladder jack scaffold platform and were not provided an independent means to access the scaffold platform.

(a) Location - 11 Broadway Avenue, Rockport, MA:  On or about 4/27/2015, an employee stripping off a porch roof was provided with a ladder that did not extend three feet beyond the roof that the employee was working.
