29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about April 29, employees were exposed to a fall distance of approximately 31 feet to the concrete while installing a window on the third story of a residential home without fall protection. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(6): Unsecured ladder was used on surfaces that were not stable and level. On or about April 29, a ladder used to access the second story of a residential home renovation was not on a level surface exposing an employee to a fall distance of approximately 23 feet to concrete. Abatement certification required within 10 days after abatement date. The certification shall include a statement that the abatement is complete, date completed, method of abatement, and states employees and their representatives were informed of this abatement.
