29 CFR 1926.1419(f): During operations requiring signals, the ability to transmit signals between the operator and signal person must be maintained. If that ability is interrupted at any time, the operator must safely stop operations requiring signals until it is reestablished and a proper signal is given and understood. a. 520 W. Main Street Rockaway, NJ Signaling procedures to communicate between the Telescopic Handler operator and the ground crew were not developed and or maintained thereby exposing the signaler to struck-by hazards from the load. Violation occurred on or about 04/28/2015

29 CFR 1926.1428(a): The employer of the signal person must ensure that each signal person meets the Qualification Requirements (paragraph (c) of this section) prior to giving any signals. This requirement must be met by using either Option (1) or Option (2) of this section. a. 520 W. Main Street Rockaway, NJ Workers engaged in signaling operations were not adequately trained in recognizing and mitigating the hazards encountered while performing signal operations. The training deficiencies include, but are not limited too; knowledge and understanding of the type of signals used. Violation occurred on or about 04/28/2015
