29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) At 6 Woodchase Avenue in Farmington, MO - On or about 04/27/2015, employees were working on a two-story residential roof (10-in-12 pitch or steeper) where employees on the first roof level were exposed to overhead hazards, such as falling roofing tools and/or materials, from the top roof level and were not protected by protective helmets (hard hats).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) At 6 Woodchase Avenue in Farmington, MO - On or about 04/27/2015, employees were working on a two-story residential roof (10-in-12 pitch or steeper) approximately 16ft above a lower solid concrete drive-way and sidewalk without having any fall protection system in place, exposing employees to elevated fall hazards. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 6 Woodchase Avenue in Farmington, MO - On or about 04/27/2015, employees were using a "Werner" 24ft aluminum extension ladder to access an upper landing surface (roof top), approximately 12ft above an earth surface, and the ladder side rails only extended one and one-half feet above the upper landing surface being accessed, exposing employees to elevated fall hazards.
