29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees:  a) Jobsite, 3709 N. Broad St., Philadelphia, Pa - Employees replacing a broken sanitary line in a trench/excavation that was approximately five (5) feet to eight (8) feet in depth and three (3) feet to twenty-two (22) feet in width were exposed to electrical hazards. The underground electrical lines were not protected to prevent contact. Condition noted on/about 4/23/15.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet  or more in depth so as to require no more than 25 feet of lateral travel for employees:  a) Jobsite, 3709 N. Broad St., Philadelphia, Pa - The employer did not provide a safe means of egress for the employees working in an excavation that was approximately five (5) feet to eight (8) feet in depth, three (3) feet to twenty-two (22) feet in width and  fifty-five (55) feet in length exposing the employees to trip, fall and cave-in hazards.  Condition observed on/about 4/23/15.

Broad St., Philadelphia, Pa - The employees were working in an excavation that was approximately five (5) feet to eight (8) feet in depth and three (3) feet to twenty-two (22) feet in width in which the pavement was undermined and a support system was not installed to protect the employees from a possible collapse.  This hazard exposed the employees to a struck-by hazards.  Condition was observed on/about 4/23/2015.
