29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:      a)  780 Old New Brunswick Rd., Somerset, NJ 08873:  Employees performing siding operations using an aluminum extension ladder that was not secured and did not extend 3 feet above the working surface were exposed to fall hazards as they accessed the landing.          Conditions on or about 4/28/2015.
