29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) On or about 04/28/2015 - Production Department, the Line #4 Drum Start-Up Procedure did not address locking out the drums prior to threading the wall base material between the forming drums during the start-up of the extruder machine exposing employees to amputation hazards.
