29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  a)  Worksite - on or about April 22, 2015, employees working on electrical systems energized to 277 and 440 volts AC, were not provided the necessary arc blast/flash rated or electrically protective equipment such as, but not limited to, eye/face protection, fire resistant over-garments, head protection, insulated tools, and voltage meters to prevent severe injury in the event of accidental electrical contact or system failure.

29 CFR 1926.403(i)(2): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures:  a)  Worksite, Basement - on or about April 22, 2015, employees were working in a main electrical control panel with the front cover removed and partial dead front removed, exposing employees to energized circuits and a shock hazard of 277/440 volts AC.  b) Worksite, Refrigerator Unit - on or about April 21, 2015, an employee was installing a refrigerator unit where the supplying 277 volt AC circuit was energized.

29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit.  The employer did not post and maintain proper warning signs where such a circuit exists:  a)  Worksite, Refrigerator Unit - on or about April 21, 2015, an employee working in an electrical junction box, was exposed to circuits energized to approximately 277 volts AC, and which had not been tested to ensure an absence of voltage prior to beginning physical contact with the components and circuits were not marked with identification tags, markings or signs to ensure employees didn't physically contact the energized circuit.
