29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Note: to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.   Location: 2 Dempsey Lane, Greenwich, CT 06831  Each employee engaged in conducting sealing operations (waterproofing) from the basket of an aerial lift was not tied off to the basket of the lift with a lanyard.

Location: 2 Dempsey Lane, Greenwich, CT 06831  Each employee, working in an aerial lift to apply conducting sealing operations (waterproofing), did not receive  training in the safe use of aerial lifts (scaffolds) by a person qualified in the subject matter in order to recognize the hazards associated with aerial lifts (scaffolds) and the procedures to minimize and/or eliminate those hazards.
