29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) Roof, residential home located at 1010 Pine Bloom Drive, Roswell, GA 30076: Employees were exposed to eye injuries while operating pneumatic Hitachi (Model NV45AB2) Coil Roofing Nail Guns at over 140 PSI without protective eyewear. Employees were exposed to metal fragments and fasteners as safety glasses or goggles were not worn.

(a) Roof, residential home located at 1010 Pine Bloom Drive, Roswell, GA 30076: Employees operated pneumatic Hitachi (Model - NV45AB2) Coil Roofing Nail Guns at 140 PSI to 185 PSI. The operating air pressure for this tool is between 70-120PSI.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Garage roof, residential home located at 1010 Pine Bloom Drive, Roswell, GA 30076: Employees were exposed to falls of approximately 10-feet to the lower level while engaged in roofing activities on a 10/12 pitched roof with no fall protection system in place.     (b) Upper roof, residential home located at 1010 Pine Bloom Drive, Roswell, GA 30076: Employees were exposed to falls of approximately 24-feet, 4-inches to the lower level while engaged in roofing activities on a 10/12 pitched roof with no fall protection system in place.     (c) Lower roof, residential home located at 1010 Pine Bloom Drive, Roswell, GA 30076: Employee was exposed to falls of approximately 10-feet to the lower level while engaged in roofing activities on a 4/12 pitched roof with no fall protection system in place.
