29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry.  (a) On April 28, 2015 the inspection revealed that employees were potentially exposed to serious injury due to the lack of a specific confined space rescue plan for entry into the 48" pipe well located at plant E & EE.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:   (a) On April 28, 2015 the inspection revealed that employees were potentially exposed to serious injury from arc flash when operating the switch on a cabinet that was not fully enclosed without personal protective equipment. Located at Plant E & EE.
