29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:   (a) On April 29, 2015 the inspection revealed that employees were potentially exposed to serious injury due to the use of a wet method tile cutting saw that was plugged into an outlet with an open ground. The hazard was located in the men's bathroom area being remodeled in building 219 at Kingsley Field in Klamath Falls, Oregon.

29 CFR 1926.405(b)(2): In an energized installation, each outlet box did not have a cover, faceplate, or fixture canopy.  (a) On April 29, 2015 the inspection revealed that employees were potentially exposed to serious injury due to outlet boxes with live electrical parts that did not have covers installed.  Uncovered outlets were located throughout building 219 at Kingsley Field in Klamath Falls, Oregon.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  (a) On April 29, 2015 the inspection revealed that the employer did not conduct training with employees on the new label elements and safety data sheet format by December 1, 2013.
