(b) Extruding Department - American Barmag Hobbs Winder Line 2: Winder machine did not have the ingoing nip points created from the lay on roller contacting the winding roller guarded, exposing employees to caught-in and crushing hazard. (c) Extruding Department - Gloucester Winder Line 3: Winder machine did not have the ingoing nip points created from the lay on roller contacting the winding roller guarded, exposing employees to caught-in and crushing hazard. (d) Extruding Department - Hobbs Winder Line 4: Winder machine did not have the ingoing nip points created from the lay on roller contacting the winding roller guarded, exposing employees to caught-in and crushing hazard. This process exposes operators to a struck-by and caught-in hazard created by the articulating pack-off wig wag. (j) Clausing Metal Lathe - Model No. 4901: Maintenance employees use the metal lathe to fabricate parts for converting equipment and other production applications. The lathe did not have a chuck guard in place, exposing maintenance employees to a caught on /caught in hazard.

(b) Compressor Room: Electrical outlet box located on the wall near the south west corner of the room did not have the unused knockout opening closed, exposing maintenance employees to an electrical shock. (c) Compressor Room: Electrical outlet box located on the west wall did not have the unused open knockout closed, exposing maintenance employees to an electrical shock.
