29 CFR 1910.107(m)(1): Spraying operations involving the use of organic peroxides and other dual component coatings were not conducted in approved sprinklered spray booths meeting the requirements of this section:  a)      HY Val Industries: On or about 04/29/2015, spray operations using flammable paint INTERTUF 262 Part A and B were not conducted in an approved spray booth equipped with a sprinkler system, thus exposing employees to possible burns.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.253(e)(3)(ii)(C)(3): Flashback protection was not provided by an approved device that would prevent flame from passing into the fuel gas system:  a)    HY Val Industries, Inc.: On or about 04/27/2015, oxygen and propylene gases were used together to remove bolts without a device to prevent flashback, as a result an employee was exposed to possible burns.  Note:  When only a portion of a fuel gas system is to be used with oxygen, only that portion needs to comply with 29 CFR 1910.253(e)(3)(i).  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.56(b)(2): Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected.  a)    HY Val Industries, Inc.: On or about 04/27/2015, a cable used in welding a valve bracket had multiple areas of exposed copper wires within 10 feet of the electrode holder, thus exposing employee to a possible electrical shock.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
