29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work:  (a) Susquehanna Valley Nursing and Rehabilitation Center, LLC - Maintenance Shop - The Sears bench grinder, serial number 82620019, was not equipped with a tool rest on the left side abrasive grinding wheel, on or about May 6, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
