29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Worksite, 190 West Sunrise Highway Freeport NY - Employees were cutting gas lines next to a pump truck that was venting gas fumes. The employees were not trained to recognize the unsafe conditions; on or about 5/5/15. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.850(e): The presence of hazardous chemicals, gases, explosives, flammable materials, or similarly dangerous substances had been used in pipes, tanks, and other equipment. Testing and purging was not performed and the hazard was not eliminated before demolition was started. a) Worksite, 190 West Sunrise Highway, Freeport NY - Employees engaged in the demolition of a gas station were using a sawsall to cut through gas lines running from dispensers (pumps) to underground tanks; on or about 5//5/15. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
