29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.    On or about May 7, 2015 the employer did not ensure that ground fault circuit interrupters or an assured equipment grounding conductor program were used during construction operations. This condition exposed employees to hazards associated with electrical shock.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.    On or about May 7, 2015 the employer did not ensure that employees operating an aerial lift were protected from falling when working 10 feet or more above ground level. This condition exposed employees to fall hazards above 10 feet.
