29 CFR 1926.100(a): An employee worked in an area where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Hu-Bran Construction, LLC @ 625-637 Bond St., St. Peters, MO 63376: On and before May 7, 2015, the employer did not ensure that employees on the worksite where flying and/or falling objects were present were equipped with head protection. Employees handed materials to an employee who worked on an elevated work platform and were subsequently exposed to falling materials and/or tools from the employee on the elevated work platform. This condition exposed employees to lacerations and/or contusions.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar structure was not used:     a) Hu-Bran Construction, LLC @ 625-637 Bond St., St. Peters, MO 63376: On and before May 7, 2015, the ladder jack scaffold located on the back side of the multi-unit, two-story, residential home was configured such that the employee had to step over the pick board and ladder jack to access the work platform. This condition exposed the employee to a fall hazard of approximately 13 feet and 4 inches when he ascended and descended the temporary elevated work platform.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    a) Hu-Bran Construction, LLC @ 625-637 Bond St., St. Peters, MO 63376: On and before May 7, 2015, the employer did not ensure that the employee who worked from the ladder jack scaffold that was located on the back side of the multi-unit, two-story, residential home was protected against falls. This condition exposed the employee to a fall hazard of approximately 13 feet and 4 inches.
