Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards:      (a) On or about May 6, 2015 and at time prior to that date, for the employees exposed to amputation hazards while operating an Amada press brake, model no. 100S, serial no. 101919 where the foot control bar was not covered to prevent inadvertent operation in the parts fabrication room at 910 East Andrew Street, Rapid City, South Dakota.      Abatement Note: Among other methods, one feasible and acceptable method to correct this hazard is to provide a guard or cover for the foot control bar that meets the requirement of the American National Standards Institute (ANSI) B11.3 - 2002 (R2007) Safety Requirements for Power Press Brakes, Section 6 Design, Construction, Reconstruction, and Modification and specifically 6.10.4 Foot Control, which states: When provided, foot controls shall be protected to prevent actuation from falling or moving objects, or from unintended actuation.

(b) On or about May 6, 2015 and at time prior to that date, for the employees exposed to amputation hazards while operating an Amada press brake, model no. 100S, serial no. 101919 in the parts fabrication room at 910 East Andrew Street, Rapid City, South Dakota.
